Step 2
Medical Documents
Obtain your medical document with the help of the TCN patient care team at 1-844-826-2665. Be sure that you print your original medical document in colour to add to your completed application.
Step 3
Health Canada Application Form
Access the full application form and instructions here for the two main types of grow at home production that’s available.
Step 4
Get Your Status
Mail your medical document and completed application in the same envelope to Health Canada’s address located on the front page of the application. To find your status, call Health Canada at 1-866-337-7705.
